The Hospice of Murray County social worker, Haley Rhode, LSW assists the patient and his/her family members in a variety of ways. The primary role of the social worker is to be an advocate for the patient throughout his/her journey. The social worker helps to develop and coordinate an individualized plan of care. The social worker also helps each patient/family with the coordination of spiritual care, bereavement, counseling, education, and links the patient/family with other, available community resources.
